How to pay

I am an in-network provider with Premera & Blue Cross/Blue Shield insurance plans, including Lifewise.

For all other insurance companies, please inquire with your plan for their reimbursement rate for out-of-network providers. 

Payment is due at the time of service. I accept credit, debit, and HSA cards. I can provide you with a superbill that you may submit to your insurance company for reimbursement.
